Project lessons learned template

This is called project lessons learn, or project closure report, or project implementation review, or project post mortem.  Whatever name might be used, objective of this template to capture major success, major issues, and relate both to project scope, budget, and success criteria defined in the charter, and stakeholders expectations.

An improvement/action plan results from lessons learned which the success of it is to be followed through to accomplish these action and more important to learn and repeat similar issues on future projects.
